Woodward Hills Health & Rehabilitation Center is seeking a Full Time Social Worker for our Bloomfield Hills, MI Skilled Nursing Facility.
Position Summary: There is a special relationship our Social Workers develop with patients and coworkers in a post-acute and long term care rehabilitation center. Our Social Workers are part of a team approach to patient care and will assist with residents and family members’ concerns & questions, make patient assessments, follow care plans, perform discharge procedures, and coordinate hospice and home care services. Candidates should have knowledge and/or previous experience with the geriatric (elder) population and State regulations as they apply to skilled nursing (SNF) and long term care experience.
Education:
- A minimum Bachelor’s Degree in Social Work
License:
- Current Social Work license as regulated by State Law
Performance Requirements:
- Knowledge, skills, and ability to perform the essential functions of the job
- Effective use of interpersonal skills
- Ability to communicate with residents, families, and interdisciplinary team members
- Initiative and flexibility in work environment
- Ability to establish and carry out priorities as an integral part of the skilled nursing team.
The LTC Social Worker is a fast-paced position requiring excellent skills in communication, listening and documentation.
Experience:
- Minimum One year experience in Skilled Nursing Facility or Long Term Care Facility
