THE IDEAL CANDIDATE
The Stanislaus County Department of Aging & Veterans Services is seeking an experienced Social Worker to provide case management for older adults that are at risk of placement or homelessness and require assistance to arrange programs and services to safely maintain their independence. THE POSITION
This Social Worker III position is the advanced journey level position for complex case management of the vulnerable elderly population. This position will act as the liaison and work closely with Adult Protective Services, In-Home Supportive Services, internal Aging and Veterans Services program staff, and various community partners to meet the needs of their clients. This position acts as a lead-worker disseminating work assignments, training staff, and handling more difficult related tasks and will perform other work as required.
The Social Worker III will work with clients who are served by the Multipurpose Senior Services Program (MSSP). The mission of MSSP is to provide comprehensive case management services for low income, frail seniors with the goal of preventing unnecessary institutionalization. The MSSP Social Worker coordinate a wide variety of services such as: adult day health care; home health services, in-home supportive services, transportation, medical equipment, and other services are needed.
Unless otherwise provided, this position is part of the Classified Service of the County and is assigned to the Community and Health Services bargaining unit for labor relations purposes. Individuals who are in a full-time classification position are required to serve a twelve-month probationary period, which may be extended an additional six months, for a total of eighteen months.
Incumbents may also be subject to overtime, standby, callback, weekend, holiday and shift assignments as identified in their MOU (Memorandum of Understanding).

Typical Tasks

 

  • Conduct psychosocial assessments and reassessments in client’s home inclusive of their social/emotional issues and practical arrangements related to the client’s health;
  • Collaborate with the Nurse Care Manager in assessing, planning, providing initial and ongoing services for clients including the development of the client’s individualized care plan including short and long-term goals;
  • Document client contacts, progress notes, and care plan goals on an ongoing and timely basis as the program follows a medical model of service delivery where every problem that is identified must be qualitatively addressed throughout the life of the client’s case;
  • Consult with the Site Director on an on-going basis to review client’s needs, progress, and other related matters requiring clarification and guidance;
  • Collaborate with community partners for additional services and support for the clients;
  • Participate in all California Department of Aging Utilization Reviews/Quality Assurance activities; and
  • Attend trainings as directed

  • Train new Social Worker I’s and II’s;
  • Visually examine clients for bruises, lacerations and other signs of physical abuse;
  • Public speaking; and
  • Learn online computer input
KNOWLEDGE
  • Socioeconomic conditions and trends;

  • Current issues in the field of social welfare;
  • Principles of interviewing and problem-solving methodology;
  • Basic public welfare programs on the Federal, State, and local levels;

EDUCATION/EXPERIENCE
PATTERN I
  • One (1) year performing duties comparable to a Social Worker II in a public social services agency; OR
  • Two and one-half (2 ½) years performing duties as a Family Services Specialist III
PATTERN II
  • Graduation from an accredited four-year college which includes 30 units in Social Welfare, Social/Human Services, Sociology or other Behavioral Science; AND
  • Two (2) years social work casework experience in a public or private social services agency
In addition to Pattern I and II:
  • Possess and maintain a valid California Driver’s License. Employees may be required to use personal transportation during the performance of their duties.
  • Possess and maintain a valid Real ID driver license or Identification Card, valid U.S. passport or passport card, military ID or other federally approved identification to fly within the United States during the performance of their duties effective October 1, 2020.
  • Proof of liability insurance as required by the State of California. Selected candidates will be required to obtain and maintain security clearance to access federal, state and local detention facilities, schools, and hospitals in the performance of their duties.

DESIRABLE QUALIFICATIONS
  • Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or related field, plus two years of experience working with frail older adults.

EXAMINATION PROCESS

  • Eligible Lists. Candidates who pass the examination will be placed on an eligible list for that classification. Eligible lists are effective for six months, but may be extended up to eighteen months.
