Job Overview

Miami Jewish Health is one of the largest healthcare providers in the southeast focused on our aging population. Our unique continuum of care on our 20+ acre campus located in the Buena Vista neighborhood of Miami, offers services that include short and long-term skilled nursing, a state-of-the art on-site dialysis center in partnership with DaVita, a 32-bed acute-care hospital, an ambulatory health clinic, an inpatient hospice unit in partnership with Seasons Hospice, a memory and research institute specializing in the treatment and support of those living with Alzheimer’s disease and other neurocognitive disorders, a multidisciplinary functional restoration program for the treatment of chronic pain, home health and home care services, and assisted and independent living. Additionally, we have the largest Program for All-Inclusive Care for the Elderly (PACE) in the state of Florida, with three locations in Dade County and one in Broward County.

 

Job Summary

The Social Worker is responsible for providing support and assistance to the PACE Social Work and Behavioral Department. Provides such social services support as Annual Certification, Case Management, Behavioral monitoring, and PSR/CAP groups.

 

Essential Job Functions

  • Supports counsels and acts as an advocate for residents and family
  • Completes all necessary and required social work documentation
  • Maintains current written case management records, including periodic reassessments of program participants
  • Interfaces with interdisciplinary staff members to share information on the resident
  • Assists with discharge planning in a rehab setting
  • Facilitates support groups
  • Attends meetings as needed

 

Job Requirements

Education

  • Bachelor’s degree in social work or closely related field

Experience

  • Six months’ minimum of related experience

Licenses/Certifications

  • Florida DOEA 701-B Comprehensive Assessment experience or certificate for PACE positions

 

Abilities Required

  • Excellent assessment and counseling skills
  • Ability to work in a fast-paced social work position
  • Must be computer literate
  • Excellent communication and interpersonal skills
  • Knowledge of geriatrics/gerontology preferred
  • Ability to work closely and well with other disciplines
  • Thorough knowledge of psychosocial dynamics of individual & family

  • Normal working condition with adequate lighting and ventilation

Infectious Material Exposure

  • No routine exposure to blood/body fluids/tissues but may occasionally require performing unplanned activities which may involve exposure to blood/body fluids/tissues.
