Social Worker – Part Time

10 am to 2 pm – Monday to Friday – No weekends.

As our member of our team, you will have the opportunity to help set the standard, providing comfort and support to our residents and their families. We are seeking a talented social worker who values older adults, to advocate and help guide our team to provide outstanding customer service.
As a member of the Social Services team your duties would include:
- Conduct MDS Assessments
- Help to develop Care Plans
- Participate in Resident Care Plan Meetings
- Providing a welcoming atmosphere and to ease the transition to our facility.
- Maintaining frequent contact with residents and their families.
- Work to optimize resident safety and acceptance.
You will use your assessment skills, make decisions, and help the team to serve our residents.

Requirements:
- At least two years’ experience in a long-term care setting
- Experience with MDS & the assessment process.
- Bachelor of Social Work is preferred.
- Genuine concern for the needs of older adults.
- Strong customer service skills with both internal & external customers.
This is a part time position. 10 am to 2 pm. Monday to Friday. No Weekends.
