Position Overview: A Supportive Housing Case Manager should be able to make mature judgements, demonstrate leadership, possess good communication skills, and maintain effective interpersonal relationships with Aging Services Of North Central Massachusetts Home Care staff, and consumers.

Responsibilities include: Identify residents with needs and assist them with securing information and refer them to appropriate service providers. Provide outreach, advocacy, and crisis intervention accordingly. Act as the case manager to those residents receiving home care program services. Plan and schedule on-going activities for the residents of the housing authority; provide educational trainings on topics of interest to residents. Facilitate monthly meetings with the housing authority representative and provider representative to monitor resident issues, needs, and services, and identifying and referring residents for other appropriate programs. Advocating effectively on behalf of elder applicants and clients to assist them in availing themselves of wide range of public and private benefits and programs.

Desired Bachelor’s degree. The Worker must be able to work with a diverse population, make mature judgements, possess good communication skills and maintain effective interpersonal relationships. Knowledge of basic medical, mental health, social service delivery systems, as well as acceptable record keeping practices is essential.

A valid driver’s license and a legally insured and registered motor vehicle are required, a copy of current license and registration will be on file in the personnel record.

Compensation and Benefits:

This is Monday-Friday, 35 hour-per-week position with competitive pay, mileage reimbursement, excellent work environment and an outstanding benefits package, including health, dental, vision, company paid life insurance, and health reimbursement account. Additionally, this position offers a 403B retirement plan, generous paid time off, and employee referral bonuses. Upon hire you will receive 6 days personal time and accrue vacation immediately.
