We are looking to employ clinicians to help clients address unhealthy and debilitating behaviors as well as overcome phobias, addictions, and traumas using the Reaching Recovery model.
The clinician’s responsibilities include helping clients understand their behaviors, teaching clients how to change their responses to unfavorable situations, and assisting clients in developing healthier coping mechanisms. You should also be able to create personalized exercises and homework assignments to help clients adjust their thought patterns.

Responsibilities:


GENERAL SUMMARY
The Transition Support Program (TSP) is a community-based outreach team that works with clients in the first 90 days after involuntary psychiatric hospitalizations to assist in making connections to outpatient care. TSP Clinicians work intensively with a small caseload of clients, with a supportive team.
TSP clinicians become true resource experts, as they work with clients using harm reduction strategies, motivational interviewing, and solution-focused interventions to move toward positive care transitions that reduce repeat hospitalizations.
Building on the mantra of “Teach, don’t take over,” TSP Clinicians provide supportive contact with clients while they are hospitalized – helping clients develop their own goals, and assist in taking achievable steps toward meeting those goals.
TSP Clinicians promote client self-advocacy and engagement in community services by providing education, coaching, and resources to promote ongoing self-management of clients’ medical and behavioral health conditions.
This is a rewarding and self-directed Job and services happen where the participants live and spend their time. This Job requires a person with a high level of comfort working with people who are experiencing a wide range of mental and/or behavioral disorders including actively using substances.

ESSENTIAL FUNCTIONS AND DUTIES
  • Assists in consulting with other team members, interns and volunteers, and reviews intakes and treatment plans.
  • Provides assessment, direct services and/or internal and external organization referrals for clients.
  • Develops and provides client assessments and treatment plans and maintains accurate and timely clinical records according to organization policy.
  • Prepares, Reviews and Audits necessary reports, summaries and progress notes, and incorporates into clinical records.
  • Performs consultation and liaison duties with other agencies and resources on behalf of the client.
  • Assists client to remain stable in the community, thereby reducing the use of hospitals or involuntary treatment services.
  • Monitors medication regimes and treatment compliance.
  • Monitors and may review solutions surrounding utilization of services and the success of outcome measures.
  • Develop plans to address poor utilization of services with Supervisor.
  • Narrow the knowledge gap between providers by helping team members gain skills needed to become co-capable and/or co-credentialed. Specific skills sets include:
    • Early detection, identification and treatment of co-occurring disorders.
    • Recognition of stages of changes and utilization of motivational interviewing to encourage movement towards reaching recovery
    • Understanding how the six dimensions of ASAM criteria are used to create holistic, biopsychosocial assessments that guide treatment planning processes across all services and levels of care.
    • Development of integrated mental health and substance use treatment plans
    • How to make appropriate referrals to partner providers that will fit client need.
    • How to effectively advocate for client needs when coordinating care with treatment provider’s mental health services.
    • Participates in team member meetings, consultations, case presentations and in-service training.
  • Performs other duties as assigned.

KNOWLEDGE, ABILITIES, AND SKILLS
  • Has an advanced level of understanding of the concepts of individual and Group therapy evidenced-based practices to provide services.
  • Possesses and routinely demonstrates a strong working knowledge of the mental and/or behavioral health and support resources of King County and has a working knowledge with state and local health delivery systems, including knowledge of various funding principles and clinical services mandates.
  • Understands recovery concepts in behavioral health; synthesizes and applies, as appropriate to their work at Sound.
  • Demonstrates the ability to learn new tasks and skills quickly, and to manage and prioritize multiple tasks, while simultaneously managing crises.
  • Have the ability to mentor and train others.
  • Ability to demonstrate the capacity to appreciate and work well with the complex and diverse population.

 

Qualifications:

EDUCATION AND EXPERIENCE
  • Master’s degree in social work, psychology, or a counseling-related field, with a strong emphasis on working with families and community agencies required.
  • A minimum of two years of clinical experience as a clinician or case manager and demonstrated skills in clinical assessment and treatment plans, preferred.
CREDENTIALS
  • Washington State driver’s license and insurable driving record required.
  • Mental Health Professional (MHP) Designation required.
  • Must be able to obtain a WA State Agency Affiliated Counselor credential within the first 60 days of hire.
  • Must pass a criminal background check.
