Job Overview

The Transitions Program is an innovative curriculum developed by experts who believe that students are most successful when purposefully engaged in self-directed and experiential learning. Glade Run’s staff work with a student’s IEP team to develop and implement a transition plan. Parents also play an integral role in tailoring the program to the individual. Students are provided with guidance as they engage in skill development, meet employment goals, and improve independent living skills.

This provides support in the areas of:

  • post-secondary education
  • post-secondary career
  • independent living skills
  • parent education

Position Summary: The primary aim of the Transition Coordinator is to maximize the quality of support provided to transition-aged students. The Transition Coordinator is responsible for general oversight of the Transitions/Independent Living Experience (ILE) Programs and will maintain a caseload. In addition to upholding the operations of the program, the Transition Coordinator is responsible for knowing and maintaining a positive rapport with each student in his/her caseload as well as each student’s extended network of support. The Transition Coordinator will work in cooperation with the Transition/ILE Programs staff, other Glade Run staff and all relevant community members to insure that services are being delivered for each student in accordance with his/her needs as defined by goal setting conference and ongoing needs assessments

REQUIREMENTS
  • Bachelor’s degree in Education or related field required.
  • Pennsylvania Teaching Certification in area of Special Education– Highly-qualified status preferred
  • Two (2) to five (5) years’ experience working with 14-18 year old individuals with special needs in transition work related educational, vocational, and/or day habilitation programs required.
  • Knowledge of ODP regulations and programs.
  • Exceptional organizational and communication skills.
  • Experience administering and interpreting skills assessments.
  • Willingness to participate in a collaborative work environment
  • Valid driver’s license and acceptable driving record is required.
  • Flexibility in work schedule.
  • Eligible for PA State Police clearance, PA Child Abuse History clearance and FBI clearance.
  • Must complete and pass agency Crisis/Physical Intervention training.

CORE COMPETENCIES:

  • Participates in Regional Marketing/Educational Opportunities on behalf of Glade Run Transitions/ILE Programs.
  • Provides oversight and accountability for the operations of the Glade Run Transitions/ILE Programs.
  • Interview and assess incoming students to determine enrollment.
  • Meet on a regularly scheduled basis with each student on caseload according to Glade Run Transitions/ILE Programs.
  • Administer and update each student’s Transitions plan in accordance with IEP team or more frequently, in accordance with student need.
  • Maintaining program enrollment that meets budgeted targets.
  • Plan/manage Office of Developmental Programs under Home and Community Based Services waiver compliance and provider qualification compliance.

POPULATION SERVED:
School district students ages 14-21 years old with Autism Spectrum Disorder and related disabilities such as an Intellectual Disability, Downs Syndrome etc
