Job Overview

Position Summary: Assesses clients’ needs and eligibility for in-home services, entitlements and benefits, and manages ongoing case work and care plan management.

Principal Responsibilities:

1. Conduct intakes and screen clients with presenting problems, if required.

2. Provide accurate and appropriate information on benefits, entitlements, programs, and case assistance and assist in accessing services, including referral, counseling clients and advocating and intervening with other agencies on behalf of clients.

3. Apply and recertify clients for entitlements and benefits.

4. Arrange for the provision of services.

5. Use language that expresses respect, patience and understanding when communicating with clients, families and collaterals.

6. Fully utilize the program’s data system to maintain complete and accurate case notes, monitor reassessment, client data and contacts, service delivery and financial information.

7. Prepare and maintain required documentation in case file.

8. Compile statistics, prepare reports and other documentation, and disseminate information when necessary

9. Actively participate in supervision and staff meetings.

10. Exchange information in a timely manner.

11. Complete work assignments thoroughly, accurately and within organizational time frames.

12. Assess clients’ social, emotional and environmental status in their home, hospital or institutional setting.

13. Develop, implement and follow up on a care plan for clients.

14. Provide continuing contact with clients.

15. Provide supportive counseling.

16. Supervise social work students, as needed.

17. Arrange for translation services, as needed, for clients.

18. Perform other duties as needed or requested

 

Knowledge, Skills And Requirements
  • Masters in Social Work required
  • Bilingual preferred
  • Applicant must be able to travel to client’s homes using public transportation or driving.
  • Experience with DFTA funded case management standards a plus.
  • Knowledge of entitlements for seniors a plus.
  • Experience with STARS data system and E-file system a plus.
  • Experience with Word and excel a plus.

 


To ensure the safety of our clients and staff, all new employees must be fully vaccinated prior to their start date. Upon acceptance of a conditional job offer, candidates who are not already vaccinated must obtain the first dose of the vaccine within three (3) calendar days of job offer and must obtain any subsequent doses in accordance with the vaccine protocol.
