Fraser is seeking a Intake & Operations Coordinator to provide excellent & caring customer service while ensuring all operations procedures are met and all safety, health, and facility procedures are carried out efficiently and effectively. This position coordinates intake functions for our Case Management, Housing, and Employment divisions, and will provide program information & other community resources to individuals over the phone and occasionally in person.
We are also offering a $500 Hiring Bonus for this position! The hiring bonus applies to new, external hires only.
Responsibilities:

  • Responsible for all intake calls for Fraser’s Case Management, Housing, and Employment divisions
  • Provides caring and professional customer service, both over the phone and in person
  • Acts as a resource to all individuals, and communicate all options and services available to them at Fraser, as well as their waiver/ funding eligibility and benefit coverage
  • Independently coordinate intake process for all new individuals, including; distributing, collecting, and processing of all enrollment forms and documents
  • Schedules appointments and assists with caseload assignments
  • Collaborates with the appropriate staff member to schedule pre-intake meetings
  • Partners with other departments as needed/requested
  • Maintain waitlists by monitoring caseload openings and contacting individuals to initiate intake process
  • Provides administrative support, including composition of written correspondence, scheduling meetings, data entry, word processing, and special projects
  • Applies postage to outgoing mail, track postage meter and order more postage when needed
  • Assist with keeping office equipment maintained and call for servicing when needed
  • Ensures daily facilities or operations needs are met
  • Works with Assistant Operations Manager and/or Facilities to address more complex needs, ensuring Fraser sites are in good working order to serve clients safely and appropriately
  • Assures adherence to all infection control, safety and program policies related to operations procedures
  • This includes maintaining supplies within first aid/emergency kits, and/or following appropriate cleaning protocol for bodily fluids and or other spills as needed
  • Manages supply inventory and coordinates supply orders for assigned sites
  • Oversees copy machine functions and coordinates repairs with IT and vendors as needed
  • Conducts or assists with emergency drills as directed by Assistant Operations Manager
  • Ensures the site has current policy/procedure(s) available for staff reference with regard to emergency response; participates as member of emergency response team
  • Monitors building exterior perimeter for cleanliness and safety issues, including keeping all entrances accessible and free of hazards
  • Works collaboratively with Assistant Operations Manager and/or Facilities to coordinate site-specific key fob management and security access
  • Other duties as assigned

Location and Schedule:

  • Fraser Bloomington
  • M-F, Full Time, 8:30am-5pm

Requirements:

  • Bachelor’s degree in Sociology, Psychology or related field or equivalent educational and experience match preferred.
  • Experience in social services or human services preferred.
  • Valid Minnesota Driver’s License with acceptable record required.
  • Ability to pass MN DHS background study and motor vehicle background study.
  • Previous intake and/or scheduling experience preferred.
  • Previous experience with insurance & the mental health and/or disability fields preferred.
  • Demonstrated ability in working with a wide variety of people and the ability to maintain positive relationships required.
  • Ability to analyze facts and independently problem solve required.
  • Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice, internal self-awareness and reflection.
  • Effective communication in the English language, including verbal and written communication with both internal and external contacts required.
  • Ability to understand and implement all related regulations, policies and procedures required.
  • Ability to recognize problems, establish facts, and draw valid conclusions required.
  • Computer proficiency using Word and Excel with proficiency in keyboarding skills required.
  • Excellent organizational skills and attention to detail required.
