  • Job Summary
    • The Social Services Coordinator for Integrated Maternal Health Program is an essential member of the perinatal collaborative care team, including obstetrical care providers, psychiatric consultant, mental health therapist, case managers, and health educators. This individual completes biopsychosocial assessment for women during the perinatal period and coordinates services delivered to patients on an assigned case load, as well as monitors symptoms of patients participating in perinatal mental health services. The Coordinator supervises care navigators and social workers who provide mental health screening and case management services. This individual ensures seamless service delivery and related community programming, including creating procedures related to the delivery of social services and staff training. With guidance from program leadership, she/he/they works collaboratively with the clinical services team to develop and implement effective systems for social service supports and integrated behavioral health care during the perinatal period. The Coordinator also oversees reporting functions and facilitates collaboration with external community partners.
  • Minimum Qualifications
    • Education/Training
      • Master of Social Work from a program accredited by the Council on Social Work Education is required.
    • Experience
      • Approximately three to five years of progressively more responsible professional roles that included case management, biopsychosocial assessment, and group facilitation. Experience with team supervision, data management, and providing social service needs during the perinatal period preferred.
    • On The Job Experience
      • Approximately six to twelve months to gain familiarity with the Hospital environment, department routines and procedures, and the full range of job duties.
    • License/Certification/Registration
      • District of Columbia Licensed Independent Clinical Social Worker (LICSW) is required.
    • Knowledge, Skills & Abilities
      • PROBLEM SOLVING: Job requires a relatively high level of problem solving to find solutions to difficult human, technical, clinical or administrative problems.

        INTERNAL CONTACTS: Internal contacts necessary to effectively advise, consult or counsel others to gain cooperation and acceptance of ideas or actions.

        INDEPENDENT JUDGEMENT: Work assignments are received in the form of results expected, due dates, and general approach to be taken. Ability to incorporate clinical skills, theoretic concepts and knowledge of costs in determining effective and appropriate course of action.

  • Primary Duties and Responsibilities
    • 1. Provides oversight and monitoring of behavioral health screening process.

      2. Conducts biopsychosocial assessments (brief clinical assessment to establish probable diagnosis, risk of harm, level of need, and preliminary treatment plan) for women with positive behavioral health screens.

      3. Coordinates initiation of appropriate level of behavioral health treatment with therapist or psychiatric supports.

      4. Coordinates regular case conferences among the collaborative care team to ensure effective service delivery.

      5. Delivers direct services to patients on an assigned case load, which may include case management, brief evidence-based therapy support, and group facilitation.

      6. Provides oversight and monitoring of all patients receiving therapy and psychiatric supports during the perinatal period.

      7. Provides direct supervision for the social services team, including social workers, care navigators, students and other associates in related roles. Maintains oversight of all case management services delivered by this team and documentation of services provided.

      8. Maintains compliance with all organizational policies and legal regulations related to delivery of perinatal social services.

      9. Tracks social service outcomes. Ensures patient outcome measures are achieved.

      10. Implements policies and procedures to ensure high quality service delivery and effective utilization of program resources. Performs regular quality assurance activities to ensure social services are delivered as intended and clients receive optimal services.

      11. Provides professional development education to promote skill development of social services team.

      12. Serves as liaison between social services team, health care providers, and behavioral health team. Facilitates communication of behavioral health and social service goals, objectives and activities across service providers.

      13. Facilitates the ongoing enrichment and growth of social and behavioral health service implementation to ensure the strongest delivery of high-impact services.

      14. Coordinates and assigns staff representatives for meetings and conferences.

      15. Serves as a point person for communication and collaboration with community partners related to patient care.

      16. Participates in the assessment and implementation of all activities necessary to ensure compliance with regulatory standards.
