Job Overview

The Askia Clinical Case Manager has the responsibility of providing case management services, supportive counseling, crisis prevention and intervention at a 16-bed co-occurring enhanced residential rehabilitation facility, affiliated with The Dimock Center. This position is responsible for ensuring that patients receive trauma-informed and culturally-sensitive care. The Clinical Case Manager is responsible for leading engagement, and connecting patients to programs and resources in the community that support reintegration and rehabilitation, with a focus on patient vocational, educational and housing needs. The Clinical Case Manager will assist in risk and safety assessment and planning; creation of collaborative treatment plans; and aftercare coordination for population of patients with entering treatment with a co-occurring mental health and substance use disorder. The Clinical Case Manager provides clinical guidance and support to team members while acting as an advocate for patients. The Clinical Case Manager provides psychoeducation, individual and group counseling to patients, utilizing evidenced-based practices.

Essential Functions

1. Demonstrates ability to establish and maintain therapeutic relationships with patient; consistently demonstrates respect for patients and commitment to their overall growth.

2. Conducts comprehensive assessment of patient’s ongoing needs.

3. Provides referrals and linkages to all necessary services and resources.

4. Provides comprehensive coordination of care with Dimock and outside services.

5. Approaches coordination of care with a whole health perspective by prioritizing connecting patients with Dimock primary care and other necessary medical services, as well as assisting the client to move along/progress through the SUD continuum of care.

6. Prepares the Aftercare Plan/Discharge Plan in collaboration with patient.

7. Attends all required staff meetings, MDT, case reviews, and clinical planning meetings.

8. Provides individual case management services and group psychoeducational/therapy groups multiple times per week.

9. Collaborates effectively with members of the treatment team and other service providers to evaluate the needs of patients, prioritizing patient safety and equity.

10. Completes all required chart documentation in an accurate and comprehensive manner, in accordance with required timeframes.

11. Documents all care coordination with the patient and other providers.

12. Collaborates on an ongoing and regular basis with mental health, substance use and healthcare treatment providers at Dimock, and in the community, including: primary care specialists; patient families and therapeutic supports; and state and local agencies and programs, which support a patient’s treatment.

13. Adheres to Dimock’s Policies and Procedures.

14. Attends trainings as assigned. Attends regular supervision meetings.

15. Develops and maintains respectful, effective and professional working relationships with other Dimock staff.

16. Demonstrates effective communication regarding all care coordination/case management activities to the Askia treatment team.

17. Facilitates a diverse set of evidence-based and psychoeducational groups that are trauma-informed and support a client’s holistic treatment.

18. Maintains professional and respectful boundaries with patients.

19. Performs all job functions in cooperation with supervisor, other members of the case management team, Recovery Specialists and other service providers involved in the treatment effort.

20. Returns all phone calls to providers and patient collaterals within 24 hours of receiving phone call; when absent, leaves instructions to contact supervisor.

21. Demonstrates a working knowledge of Dimock’s continuum of care and facilitation of appropriate referrals/contacts within the larger system.

22. Other duties as assigned.

4. Possesses excellent verbal and written communication skills.

5. Possesses intermediate computer skills (Microsoft: Word, Excel, Outlook, PowerPoint).

Work Environment

This position operates in a clinical/office setting.

Physical Demands

This position requires the ability to lift and carry up to 10 pounds. Frequent standing and walking required.

Position Type/Expected Hours of Work

This position operates on a 37.5-hour workweek. Occasional evenings and weekends may be required.

Required Education and Experience

1. Master’s Degree in social work, counseling/psychology, education or related field required.

2. Experience providing case management services preferred.

3. Experience working as a professional in a substance use treatment and/or residential environment preferred.
