Up to $1500 Bonus

  • $500 New Hire Bonus (New Hire Bonus paid within first full pay period paycheck)

Woodfords Family Services has an opening for a full-time Shared Living Case Manager within our Shared Living Program:

  • Woodfords’ Shared Living Program is designed to provide family, home-based services to adults age 18 and older with intellectual and/or developmental disabilities who are not able to live independently. An alternative to group home living, adults are matched with provider families who provide stable support systems and natural family environments that foster relationship building and community inclusion.

Qualifications of the Shared Living Case Manager:

  • Candidate must possess a minimum of a high school diploma or equivalent, although an Associate’s Degree or Bachelor’s Degree is preferred (A candidate actively pursuing their degree each semester considered if they possess multiple years of professional experience in the field)
  • Must possess at least two (2) years of experience in the field and have knowledge of intellectual and developmental disabilities and delays and mental health challenges
  • Must possess a valid State of Maine Driver’s License, an acceptable background check and have use of a properly insured, and dependable vehicle
  • Must possess excellent organization skills
  • Must possess excellent communication skills, including written, verbal and interpersonal skills
  • Must possess the ability to observe, interpret and document each consumer’s progress according to each consumer’s individualized service plan
  • Must possess the ability to develop a positive and healthy rapport with consumers and their families


Responsibilities of the Shared Living Case Manager:

  • Case Management of open established Shared Living Provider homes
  • Serve as a field supervisor for Direct Support Professionals (DSPs) and Shared Living Providers
  • Meet with Shared Living Providers, Direct Support Professional staff (DSPs), community partners and providers, consumers’ families and consumers at least once monthly
  • Maintain contact with staff (DSPs), Shared Living Providers, consumers and their families via phone, email and general mail as needed
  • Write service plans and ensure they are distributed to the team and consumer
  • Serve as liaison between consumers and service providers utilized by the consumer (public schools, medical providers, psychiatrists, day programming, etc.)
  • Be on-call for Shared Living Providers, staff (DSPs), and consumers to provide support in case of an emergency
