Position: Clinical Outreach Manager (Office-Based)

The Monogram Health Clinical Outreach & Performance Manager will have the opportunity to improve patient lives each and every day. This clinician will complete ad-hoc telephonic visits with members and work on corporate clinical initiatives, as assigned. He or she will partner with the enrollment & outreach team to contact patients who have previously missed a scheduled home visit and assist in completing comprehensive health assessments (CHA) and scheduling a timely subsequent in-home appointment with their assigned Care Manager. Monogram is first and foremost an in-home care management program; these limited telephonic visits are meant to serve as engagement tools for patients who miss initial visits. The purpose is to get the hard-to-reach patient initially engaged with a CHA, as a bridge to in-home, in-person care management.

Additionally, this individual will work with company Clinical Performance Managers and take ownership of key company-wide clinical performance measures. This includes measuring and reporting on regional and individual performance and assisting individuals to improving outcomes. Examples include helping clinical operations colleagues monitor transplant eligible patients and drive transplant referrals.

Position Requirements

· The position will be located in the company’s Tempe, AZ or Nashville, TN office

· Graduate of an accredited School of Nursing

· Current Compact Licensure as a Registered Nurse

· Ability to show empathy and quickly build relationships with patients and colleagues

· 3+ years previous experience working in managed care, care management, home health and/or with CKD/ESRD patients

· Excellent verbal communication skills both in person and on the phone

· Capacity to adapt to schedule changes with short notice; multi-tasking ability is a must

· Sharp clinical documentation skills

Roles and Responsibilities

· Perform telephonic visits and assessments daily with members who have kidney disease and multiple co-morbidities, with the aim of engaging the patient in a proactive plan of care and assisting in scheduling timely subsequent in-home appointments

· Interact daily with members who suffer from challenging physical health, emotional health and socioeconomic conditions, with compassion and patience

· Drive performance in key company-wide clinical measures such as transplant evaluation, advance care planning, and/or nephrologist engagement

· Utilize reporting and analytics to identify areas where improvement is needed and drive clinical outcomes

· Support Monogram Health as necessary with any tasks or projects required to deliver excellent personalized kidney care and perform all other duties as assigned
