ABOUT THE POSITION: This position works with incarcerated or otherwise legally involved individuals who are diagnosed with mental illness through their transition into treatment and services in the community.
SPECIFIC RESPONSIBILITIES INCLUDE:
Service System Performance Management

  • Assist fellow AOT staff with reviewing referrals for AOT criteria determination
  • Monitor compliance with AOT court orders and voluntary agreements, with written and in person follow-up with client and provider
  • Interact with and participate in discharge, family, and treatment team meetings (with Hospital, outpatient, family, jail, etc.) to reinforce AOT involvement
  • Formulate petitions and voluntary agreements in timely manner
  • Assist AOT Coordinator in monitoring clients with active AOT orders who are released from NYS Department of Corrections and support prison systems with linkages to behavioral health services in Monroe County
  • Assist AOT Coordinator in scheduling AOT exams
  • Link all new referrals to Care Management agencies upon completion of initial AOT examination
  • Amend current orders as needed and submit for doctor review
  • Prepare reports as required by Monroe County and NYS OMH
  • Record progress notes in a comprehensive, timely manner
  • Monitor all scheduled AOT hearings as scheduled
  • Attend court related to AOT matters as necessary

System-wide Service Planning

  • Implement and assist with the development of policy/procedures to ensure that clients’ treatment needs are met
  • Communicate effectively with providers, criminal justice personnel, and families to assist clients with maintaining compliance with treatment and AOT
  • Monitor response of providers and assist with the development and implementation of performance improvement activities
  • Performs other duties as assigned.

EDUCATION AND EXPERIENCE:

  • M.S.W., M.S., or M.A. in a Mental Health, Social Work, or Human Services related area preferred, B.A or B.S. required or equivalent education and work experience (see CCSI’s equivalency chart).
  • Three to Five (3 – 5) years’ experience in mental health field

ADDITIONAL REQUIREMENTS:

  • Must possess a valid class D New York State Driver’s License and must have access to a motor vehicle in order to meet the transportation requirements of this position
  • Travel as necessary to fulfill responsibilities
  • Ability to satisfactorily complete required background screenings: SEL, State Central Registry, criminal and driving.
  • Use team approach in working with Monroe County Office of Mental Health staff and the community

  • Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance. Staff working directly with protected health information /electronically stored protected health information, personally identifiable information, and other patient/consumer/customer data, will abide by the specific procedures and policies outlined by CCSI’s Code of Conduct and CCSI’s Privacy and Security Policies, New York State regulations and Federal regulations.
