Social Worker – LMHC/LMSW

Concourse House, HDFC is a not for profit providing services to families in our Northwest Bronx community. We provide those who live here with services they need to strive as families. Those services include transitional housing, case management and child care, through our shelter, social work field staff and day care programs respectively. It is our hope that every family that we work with is able to live a healthy independent life by utilizing the resources we provide.

Our transitional shelter is home for 44 families comprised of women and their children. There we work to eliminate homelessness by providing families with safe, stable, transitional housing. We work with each
family according to their own individual needs and provide services based on those needs. We emphasizes stability and healthy independent living by providing our women and children with a supportive, engaging environment, We hope to instill the tools needed to make a successful transition into permanent housing for every family we serve.

We are seeking a full-time licensed LMHC/LMSW to serve our transitional.

Responsibilities include but are not limited to:

• completing a comprehensive biopsychosocial assessment with families as they enter the shelter to determine individual needs
• provide short-term counseling, crisis intervention, safety planning and psycho-education
• collaborate with ACS and/or preventive services agencies as needed and participate in ACS conferences
• make outpatient referrals and appropriate linkages ongoingly
• communicate and collaborate with service providers to enhance engagement and ensure quality of service
• provide education/trainings to other staff
• facilitate quarterly meetings with families and staff when needed
• facilitate group work with clients
• Assisting clients with supportive housing applications when clinical criteria is met
