Job Overview

We are looking for an Associate Mental Health and Wellness Clinician for the Psychiatric Acute Adult Department at Mission Hospital Regional Medical Center.

Location: Laguna Beach, CA

Work Schedule: Full time – 80 Biweekly Hours

Shift: 8-hour, Days

Job Summary:

Through an evidenced based treatment approach, the clinician uses a recovery model to promote the wellbeing of patients and families served. The clinician completes assessments, implements treatment plans, and evaluates the options and services required to meet the patient needs for positive outcomes. The clinician primarily provides group therapy, individual and family counseling. Additional responsibilities may include psychosocial assessments, treatment planning and discharge planning. The clinician works collaboratively with the multi-disciplinary team to meet patient treatment goals. The facilitation of patient specific legal and patient rights referral/processes may also be required of the clinician. This position may provide care in any area of the Mental Health & Wellness Clinical Institute to assist patients/families. Will practice in accordance with all regulatory, professional, legal, and hospital policies.

Essential Functions:

  • CLINICAL PRACTICE: The clinician is accountable to recognize the mental health and quality of life needs of patients, family, and community and to collaborate with them to achieve mutually agreed upon goals. The clinician utilizes his/her specialized skills and clinical knowledge to design collaborative and integrated care that binds together all resources in a goal oriented and strengths based perspective. Components of clinical practice are competence, evidenced based practice/continuous quality improvement, and practice standards.
  • Community Assessment (Outpatient Psychiatric only):
  • Completes community assessment including establishment of preliminary diagnosis.
  • Psychosocial Assessment:
  • Performs psychosocial within established timeframes.
  • Assess patient’s family, psychiatric, chemical dependency, medical, current functioning status, and psychosocial history.
  • Assessments result in goal specific treatment plan for the program milieu and post-hospital care.
  • Individual Interventions:
  • Provides fundamental counseling.
  • Identifies and provides education based on patient need.
  • Identifies the need and provides crisis intervention when necessary.
  • Provides individual solution focused brief counseling with application of evidenced based practices.
  • Provides individuals treatment objectives based on individual session outcomes.
  • Stays current on latest trends and/or methods of psychotherapy and provides updates to colleagues.
  • Facilitates certification review hearings, and all mental health legal filing & documentation; (i.e. Writ of Habeas Corpus, Riese Hearings, Temporary Conservatorships, etc.) based on unit needs.
  • Family Interventions:
  • Provides basic education and orientation to unit and disease process.
  • Communicates with family as indicated.
  • Group Interventions:
  • Provides fundamental disease education, 12-step process, goals group, and life skills group sessions.
  • Provides group therapy sessions including, but not limited to, disease education, psychotherapy education, coping skills, and dual diagnosis groups.
  • Facilitates broad range of chemical dependency and mental health groups with application of evidence based practices (CBT, DBT, motivational interviewing, etc.).
  • Maintains therapy curriculum routinely to keep up to date with evidence based practices.
  • Interventions / Treatment Planning:
  • Participates and provides updates on patient progress during treatment team rounds.
  • Identifies and continually applies individualized treatment team plans for patients.
  • Documents appropriately and per policy in the EMR.
  • Demonstrates knowledge and use of appropriate interventions to implement plan.
  • Communicates plan and/or changes to plan with healthcare team in a timely manner.
  • Discharge Planning:
  • Assists with discharge planning as needed.
  • Performs timely discharge planning assessments with patient and/or family promptly and according to department policies and procedures.
  • Prioritizes appropriately to organize workload.
  • Communicates and follows up on assessment with patient and family and healthcare team in a timely and meaningful manner.
  • Implements timely discharge plan to meet patient after care goals and LOS.
  • Modifies plan to address barriers.
  • Coordinates follow-up services with appropriate community agencies.
  • Maintains knowledge base of community resources for behavioral health and/or chemical dependency needs.
  • Intradepartmental referrals:
  • Collaborates with care team on referral appropriateness to outpatient programs.
  • Coordinates with outpatient program for tour of space, program and other information.
  • Outpatient clinicians provide information to inpatient population on an as needed basis.
  • Develops referrals to outpatient programs.
  • Implements safe & therapeutic care for patients:
  • Implements a safe and therapeutic plan of care for patients of minimal complexity according to the hospital’s policies and procedures.
  • Requests assistance from preceptor, or team leaders when essential care cannot be completed.
  • Detects obvious changes in-patient conditions and provides care appropriately.
  • Provides ongoing teaching to patient/family regarding treatment and programming.
  • Recognizes and responds to priorities in meeting patient care responsibilities specifically changes in patient’s condition. Incorporates clinical findings into priority setting.
  • Includes patient and family in implementing a culturally sensitive plan based upon their ability to participate and make decisions regarding care.
  • Implements plan of care for patients of moderate complexity.
  • Re-prioritizes as necessary to adapt to changing patient condition, work environment and patient assignments.
  • Responds to individual/cultural differences in patient/family care.
  • Anticipates and integrates individual/cultural differences into patient/family care.
  • Implements safe and therapeutic care to complex, high-risk patients according to hospital policies and procedures.
  • Anticipates and responds to potential problems and patient complications in complex, high-risk populations.
  • Crisis Intervention:
  • Utilize de-escalation techniques based on workplace violence training.
  • Communicate with the care team regarding the crisis and resolution and/or plan.
  • Communicate with family if appropriate.
  • Ability to assess for need of legal hold referral (LPS).
  • Knowledge of who and when to refer to when legal hold is necessary.
  • COLLABORATION: The process whereby the health care team plan and practice together as colleagues working interdependently within the boundaries of their scope of practices using shared values and mutual respect for each other’s contribution to care for individuals, their families and communities. Major elements include non-hierarchical relationships and intra-inter professional collegiality.
  • Facilitates patient care through effective communication:
  • Communicates patient condition/care changes to charge nurse, supervisor, medical staff and other team members.
  • Completes shift report and documentation in a timely manner.
  • Anticipates and is prepared to present information to patients and families regarding plan of care.
  • Responsible for accurate, complete, and individualized documentation of patient assessment, goals, plan of care, interventions and care provided, as well as patient’s progress toward discharge goals.
  • Develops & maintains therapeutic & effective relationships with patient/family & the health care team:
  • Treats others with dignity and respect and adheres to SJHS values.
  • Demonstrates knowledge and appropriate use of chain of command.
  • Educates patient/family about their plan of care across the continuum.
  • Participates in staff meetings by communicating issues and offering solutions that affect the unit.
  • Promotes collaborative and caring relationships with colleagues that contribute to teamwork and respects culturally diverse individuals and viewpoints.
  • Seeks assistance and support in unfamiliar clinical situations and utilizes the guidance of preceptors.
  • Demonstrates professional behavior with patients/families and healthcare team.
  • Aftercare for patients (i.e. aftercare groups, family groups, alumni groups, etc.):
  • Assess and refer appropriately to the aftercare programs at Mission Hospital.
  • Educate and communicate benefits of program to patients and families.
  • Demonstrates awareness of unit/ service budget:
  • Demonstrates efficient use of time, equipment, and supplies.
  • Demonstrates awareness of unit & service guidelines related to staffing & scheduling:
  • Follows scheduling guidelines and demonstrates flexibility in order to meet unit and patient needs.

  • Knowledge and expertise of psychiatric or chemical dependency disorders and treatment modalities.

  • Knowledge of community resources especially related to those serving behavioral health or chemical dependency patients.
  • Knowledge of mental health laws and patient rights.
  • Excellent interpersonal communication including conflict resolution, advocacy, problem solving and negotiation skills.
  • Good time management, independent judgment, and organizational skills.
  • Ability to manage multiple demands effectively and prioritize effectively.
  • Basic computer skills for hospital information systems.
  • Requires standing and walking 20 – 25%.
  • Proficient in required competencies upon completion of orientation as listed on unit specific competencies.

Minimum Position Requirements:

Education: Master’s Degree in Social Work or Psychology, or related field.

Experience: Less than 1 year of work experience.

Licenses/Certifications:

  • ACSW or AMFT.
  • Current American Heart Association BLS for Health Care Provider certification.
  • Crisis Prevention certification within 3 months of hire.
  • Professional Practice Model Clinical Ladder: Registered associate number with the CA Board of Behavioral Sciences.
