Job Overview

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Community Bridges is currently seeking Clinical Assessor.

Responsibilities

  • A Community Bridges, Inc. (CBI) Clinical Assessor is a Behavioral Health Technician (BHT) who has been credentialed and privileged by the designated RBHA or is a licensed professional who is responsible for conducting the initial screening, intake, and assessment with any individual who is referred to CBI for outpatient services.
  • The Clinical Assessor screens referred individuals for treatment appropriateness, reviews and completes all necessary legal documents with the patient/participant, conducts a biopsychosocial assessment, develops an initial treatment plan with each patient/participant, provides referrals based on identified needs, orients each patient/participant to his or her treatment modality, and coordinates initial services with the referring agency or other members of the treatment team.
  • Serves as the initial clinical point of contact for Outpatient Services and welcomes everyone in a respectful manner.
  • Review’s referral information with each individual and orients each individual to the intake/assessment process.
  • Is responsible for initial utilization and review activities with designated insurance companies and/or other contracted parties.
  • Ensures that appropriate medication records, treatment plans, and assessment information is faxed by the patient’s attending case manager, medical practitioner, psychiatric practitioner, or other related party.

Requirements

  • Independently Licensed Professional – Has been licensed at the independent with The Arizona Board of Behavioral
  • Bachelor’s Degree, Masters Degree or Associate Licensed Professional in field related to behavioral health and/or has been licensed at the associate level with The Arizona Board of Behavioral Health Examiners as Psychiatrist, Behavioral health Medical practitioner, Psychologist, Social worker, Counselor, Marriage and family therapist, Substance abuse counselor or Registered nurse.
  • At least one year of full-time behavioral Health work experience.
  • Paraprofessional – Has a Minimum of a high school diploma or GED. Six weeks of behavioral health work experience or receives six weeks of continuous onsite direction from a behavioral health professional, a behavioral health technician, or a behavioral health paraprofessional who has at least six months of behavioral health work experience.
  • Nurse – Currently licensed without restriction with the Arizona Board of Nursing as a Licensed Practical Nurse (LPN) or Registered Nurse according to A.R.S. Title 32, Chapter 15.
  • Emergency Medical Technician (EMT) Certification; within the State of Arizona.
  • Certified Nursing Assistant (CNA)
  • Valid Arizona Drivers License
