Social Worker

A nursing facility in Ocean County, NJ is looking to hire an experienced Social Worker to join their wonderful team!

RESPONSIBILITIES OF A SOCIAL WORKER:

• Identifies cognitive impairments, mood problems, and other psycho-social needs and follows-up as needed.
• Schedules and takes the lead in interdisciplinary meetings with residents and their families.
• Maintains accurate, thorough, and timely documentation.
• Reviews Advanced Directives and provides education to residents and families.
• Acts as a resident advocate and is familiar with residents’ rights.
• Makes appropriate referrals to outside agencies.
• Helps provide a smooth admission and discharge process for residents and families.
• Acts as a liaison for ancillary services.

The Social Worker is required to have a minimum of a Bachelor’s degree in Social Work and a current Social Worker license in good standing. A minimum of 1 year of experience required, preferably in a skilled nursing and rehab center. Other comparable healthcare experiences will also be considered.
