Position Description

This Social Worker Senior is position is located in the Family Support division of the Department of Social Services and is responsible for performing the more difficult and complex professional duties involved in assisting individuals and customers with overcoming barriers to becoming self-sufficient and having the best quality of life possible. Except for minors, the position works with all populations, including homeless individuals and families and those at risk of becoming homeless. Metro Social Services assists customers with navigating the system and obtaining information, resources, and case management services.

Metropolitan Social Services is in the business of helping Nashvillians who are experiencing hardship. Last year the department assisted more than 3600 new clients in addition to its ongoing case management customers. Services include counseling, information and referral, case management and other assistance. Typical Duties

 

  • Determines the needs and eligibility of persons seeking assistance.
  • Assesses eligibility according to program polices and guidelines.
  • Provides information and referral services to customers and the public.
  • Meets with clients on an individual, family or group basis.
  • Works in the community and performing outreach when appropriate.
  • Completes home visit to customers or to other agencies or locations in the community.
  • Maintains accurate and professional case records and contacts.
  • Completes a detailed Social Comprehensive Assessment and all needed documentation and forms completely, accurately and in a timely and ongoing manner.
  • Provides intense case management with customers experiencing mental illness (diagnosed & undiagnosed) and substance abuse.
  • Works closely with community partners in the homeless & substance abuse areas of expertise.
  • May be responsible for case management and case review of Conservatorship cases and reports.
  • Provides de-escalation techniques in a counseling environment when working with a customer and trains co-workers on de-escalation techniques.
  • Provides the customer with all the information to enable them to understand the programs services and the information needed to understand the continuation of services.
  • Provides intensive and supportive case management and referral services to and from different agency partners for necessary customers.
  • Develops, markets, maintains and nurtures community partnerships.
  • May be the agency liaison between Social Services and another community program or service.
  • May assist with developing and/or maintaining a program or service.
  • May be called upon to oversee and/or administer a program or service.
  • Works closely with Program Management and the Executive Director.
  • May be asked to represent the agency on behalf of management.

 

Minimum Qualifications

Master’s Degree in Social Work from an accredited college or university and three (3) years of experience in social work, program management, and/or program development.

Note: Per Metro Ordinance No. SO94-1078. All employees of the Metropolitan Government shall be residents of the State of Tennessee or become residents of the state within six (6) months of employment as a prerequisite to employment with the Metropolitan Government.

Preferred Experience, Knowledge, Skills, and Abilities

 

  • Knowledge of basic principles, methodologies and ethics involved in social work.
  • Knowledge of community resources
  • Experience in working with ex-felons, homeless, disabled individuals, families with children, domestic violence victims and elderly
  • Experience dealing with Mental Health and Substance Abuse services
  • Knowledge of community resources
  • Skill in in-depth interviewing, counseling with and assisting customers with goal formation and realization, and making referrals
  • Experience and/or knowledge of the Two-Generation approach and Trauma-Informed care
