JOB SUMMARY:
Assertive Community Treatment (ACT) is an evidence-based practice that offers treatment, rehabilitation, and support services, using a person-centered, recovery-based approach, to individuals that have been diagnosed with serious mental illness (SMI). The ACT team is a mobile, multi-disciplinary group of professionals who deliver the treatment and services needed in the manner and location that is most comfortable for the individual. The Clinician ACT provides clinical assessment, evaluation, and intervention skills to persons with SMI and co-occurring disorders who need intensive services within a community-based setting. Intensive service coordination and supportive services are an integral component to this position.

ESSENTIAL FUNCTIONS:

  • Conduct comprehensive assessments of psychiatric history, mental health status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
  • Provide assistance to Team Leader ACT in completion of service request authorizations and providing clinical information to insurance companies.
  • Develop and monitor person-centered, individualized treatment plans and provide ongoing assessment of treatment needs. Make appropriate changes in treatment plans and interventions based on individuals’ change in mental health status.
  • Provide individual, family, or group therapy using a variety of evidence-based modalities along with rehabilitation and support services to address symptoms, stress, engagement in leisure activities, and interpersonal issues and to assist individual in meeting treatment goals.
  • Provide individual, family, or group therapy using evidence-based strategies in order to address co-occurring disorder needs.
  • Provide crisis intervention services during regular scheduled work hours and after hours on-call (rotated among all team members) to support individuals in emergency situations by phone and/or face to face in order to reduce the acute symptoms related to the emergency and to prevent hospital admission.
  • Participate in daily clinical treatment team meetings to facilitate individual’s recovery and to provide advocacy.
  • Follow established agency methods and procedures for documentation of progress notes, individualized treatment plans, and quarterly reviews in the electronic health care record.

OTHER DUTIES:

  • Provide intensive case management services to individuals as assigned and share in provision of education needed to develop skills in areas of personal care, safety skills, money management, clothing maintenance, household maintenance, social skills, use of transportation, and accessing community resources.
  • Share duties in provision of wellness recovery strategies; vocational rehabilitation efforts; and obtaining/maintaining safe, affordable housing
  • Assist/support access to medical and dental care
  • Collaborate with individual and natural supports at least monthly
  • Provide medication delivery (i.e. daily, weekly), support, observation, monitoring, and education to the individuals. Additional medication training is required and provided by the agency to ACT staff.
  • Perform such other duties as assigned by supervisor

QUALIFICATIONS:

  • Knowledge of the Assertive Community Treatment (ACT) model
  • Knowledge of serious mental illness and co-occurring disorders
  • Skills in written and oral communication.
  • Skills working effectively with the Serious Mental Illness (SMI) and Substance Use Disorder (SUD) population
  • Skills in interviewing, assessing, decision making, and treatment planning
  • Time management and organizational skills.

Minimum Requirements

EXPERIENCE/EDUCATION REQUIRED:
Master’s degree in a human service-related field and licensed or license-eligible in the Commonwealth of Virginia or must register for supervision within ninety (90) days of starting employment.
Minimum of one (1) year clinical experience working directly with SMI and/or substance use populations
Valid driver’s license and a safe driving record.
Mount Rogers Community Services, in following current CMS regulations, requires all New Hires to be fully Vaccinated against COVID 19 or have a religious or medical exemption approved no later than the date of employment.
