Job Summary
This role provides case management services to up 30 Early Head Start families, plans/facilitates relevant therapeutic groups and participates in other program activities as appropriate. Provides direct service to families as specified in the Office of Head Start Performance Standards. Attend and participate in interagency meetings as needed. Maintain contact and coordinate with Early Head Start family to support goal developments, identify challenges and plan interventions to support successful completion of goals. Conduct regular visits to the families’ homes and community agencies servicing the families. Maintain accurate and timely documentation. Communicate with referring agencies as needed.
BSW with Proficient Microsoft Office software, customer service principles and practices; problems solving ability, and the ability to interact sensitively with at risk families. At least five (5) years of experience working with children and/or families
Job Responsibilities
Provide Case Management Services
  • Provide case management services to families including connection to community resources, medical follow-ups, substance abuse or mental health treatment, school transitions / transfers / programs, housing, recreation, legal services / support, job training / placement and parenting education / support
  • Conduct consent processes and client intakes to identify service goals
  • Complete documentation and data entry including intake and encounter / consent forms
  • Collaborate with health and human service professionals and community agencies in order to ensure compliance and comprehensive treatment to clients and families
  • Maintain regular participant interaction and contact including program meetings, court hearings, school visits, psychiatric evaluations, and client therapy sessions.
  • Arrange client transportation to attend services
  • Practice a trauma-informed approach in client interactions, both individually and in groups
  • Facilitate psychoeducational client support groups

  • Maintain relationships with and present programs to internal and external audiences
  • Stay current on local activities to leverage program activities
  • Participate in regular team meetings and case reviews
  • Identify barriers and contribute to initiatives to improve service delivery
  • Work with supervisor to refine program materials and procedures as needed
  • Other duties as assigned

DRL – Driver’s License
Required Education and Experience
Required Education: Bachelor’s degree
Required Experience:
  • Five (5) years of experience in a “at risk” community related environment
Previous experience working with children and diverse populations
Preferred Education, Experience & Cert/Lic
Preferred Education: Master’s degree
Preferred Experience:
  • Three (3) years of experience in a healthcare related environment
  • Previous Experience working with children and diverse populations
Preferred Licenses/certificates/registrations: Licensed Social Worker (LSW)

  • Basic knowledge of trauma-informed care.

To carry out its mission, it is of critical importance for the Children’s Hospital of Philadelphia (CHOP) to keep our patients, families and workforce safe and healthy and to support the health of our global community.
