Clarity HR Solutions is partnering with the Personal Involvement Center to fill the Case Manager role.

 

About the Organization

The Personal Involvement Center (PIC) is a social services agency located in LA County. PIC has served the Los Angeles and Antelope Valley communities for over 50 years. The PIC Team is dedicated to providing vital services to the children and families in their community and is looking for that same dedication in all new Team members.

 

About the Position

Case Manager is responsible for providing case management duties and outreach to assigned clients and their families with appropriate referrals and resources to assist them in sustainability.

 

  • Weekly in-agency/community setting
  • Provide community resources/linkages
  • Ensure safety/security of children by observation
  • Conduct intake assessment
  • Maintain caseload documentation in compliance with all agency polices and procedures as well as federal, state, county and local regulations
  • Provide home visits, etc.

 

Qualifications

  • Master’s Degree in a Behavioral Science
  • 1 year of Case management experience in a mental health or a community base social service agency preferred
  • Proficiency in a data entry computer database.
