Job Overview

POSITION SUMMARY

The Housing Focused Case Manager (HFCM) is responsible for integrated and coordinated case management of assigned individuals at the 801 East Men’s Shelter, which is funded by a grant agreement with the District’s Department of Human Services (DHS). The HFCM collaborates with a caseload of ~ 25 to 30 clients who are housing motivated and wish to engage in shelter exit planning. The HFCM coordinates services with other providers to maximize opportunities for success for shelter stayers, e.g., Day Services, Health Clinic, Respite staff, Shelter Operator, Food Service Vendor, etc.

Overall, the HFCM is responsible for delivering high quality, innovative, and integrated Housing Focused Case Management services. The HFCM provides supportive services and coordinates service in a manner that is culturally humble/responsive, trauma-informed, and evidence-based. The HFCM engages program participants with an attitude of “Radical Hospitality” while delivering and coordinating supportive services that promote housing acquisition.

KEY RESPONSIBILITIES INCLUDE:

  • Assists Shelter Operator team with Orientation, Screening, and Bed Placement with individuals staying in the shelter
  • Engages assigned participants with Evidence Based Practices to build trust/ develop rapport (e.g., Motivational Interviewing, Stagewise Case Management, Critical Time Intervention, Trauma-Informed Services, etc.)
  • Meets regularly with participants to review progress and support participant in achievement of housing focused goals/action items. Work may require accompanying participants to appointments, if/as needed.
  • Supports clients with identifying strengths and opportunities.
  • Engages with clients to complete a Housing Assessment and develop or update a Housing Plan (e.g., conduct Housing Preference Survey, identify barriers to obtaining housing, review realistic expectations of being matched to a subsidy based on assessment score and the District’s CAHP prioritization policy)
  • Provides In-Reach services at the shelter, recognizing that some participants will prefer to avoid service providers.
  • Ensures coordination of Housing Focused Case Management services with other providers, vendors in the building (Day Services Center, Respite Care, Health Clinic, Security, and additional service providers)

DESIRED KNOWLEDGE/SKILLS/ABILITIES INCLUDE:

  • Bachelor’s degree in related health, human services, social services or equivalent preferred
  • Minimum 2 years’ professional experience working in a related health, human service, or social services field
  • Experience working in homeless services and/or supportive housing required
  • Experience with the “Housing First” model preferred (And experience with Critical Time Intervention, Motivational Interviewing, and Stagewise Case Management also plus)
  • Experience working with adults experiencing mental health, substance use, and or trauma preferred
  • Experience with non-profit organizations a plus
