  4. Behavioral Health Care Manager – As a care manager, you are responsible for providing care management for specified CCBHC patients, health center patients, and community members. This includes intake assessments, referring patients to needed services, conducting client outreach and engagement activities, and other duties. A bachelor’s degree in social work and two years of relevant experience are required.
