  2. Registered Nurse Case Manager – As an RN Case Manager, you will plan, organize, and direct hospice care with an emphasis on community health education. You will complete comprehensive assessments, initiate plans of care, provide professional nursing care, and perform other tasks. This assignment requires 1-2 years of clinical care experience and a degree from an accredited nursing school.  
  3. Inpatient Case Manager – This position is in a 25+ bed inpatient facility where you will be responsible for ensuring patients have a smooth transition from inpatient acute care to post-acute rehabilitation programs. This position pays between $65,000 and $75,000 to start. 
  4. Clinical Case Manager – As a Clinical Case Manager, you will facilitate multiple care aspects, which include case coordination, information sharing, referral to outside resources, risk assessments, prevention treatment, and disease education. You will also help patients make informed decisions by acting as their advocate for all treatment options. This position requires a master’s degree and two years of relevant work experience. 
  5. Clinical Therapist/SED Case Manager – Some of the duties for this position include providing direct clinical services to mentally ill children, adolescents, and young adults. Other responsibilities include performing assessments, interventions, and other aspects of crisis services. This position requires LPC or LCSW certifications.
